Track: Fundamentals of Anesthesiology

NEW! Journal CME - 2018 December

Intraoperative lung-protective ventilation has been recommended to reduce postoperative pulmonary complications after abdominal surgery. Although the protective role of a more physiologic tidal volume has been established, the added protection afforded by positive end-expiratory pressure (PEEP) remains uncertain. The authors hypothesized that a low fixed PEEP might not fit all patients and that an individually titrated PEEP during anesthesia might improve lung function during and after surgery.

A-ACLS: First Response to Circulatory Crisis in the OR – 2018

Advanced Cardiac Life Support (ACLS) was originally developed as an extension of Basic Life Support (BLS) and focused on the resuscitation of individuals found unresponsive in the field. ACLS has been expanded to include immediate care in the emergency department and patients found unresponsive anywhere in the hospital. A-ACLS was created to address the unique aspects of the perioperative/periprocedural setting.  Originally created in 2015, this course has been updated to reflect the changes in the 2017 American Heart Association ACLS Guidelines, and updates to the original A-ACLS guidelines. The course includes multiple scenarios where participants have the opportunity to make "first response" decisions to cardiac situations in the OR.

Journal CME – 2018 August

A ß-LACTAM allergy is the most common suspected inhospital drug allergy, with an incidence of 5 to 17% in hospitalized patients and up to 35% in the surgical population at the preoperative assessment clinic.1–5 Thus, the team in the operating theater will be confronted with these patients when perioperative antibiotic prophylaxis is needed. Frequently, the consequence of a presumed ß-lactam allergy is that all ß-lactam antibiotics are avoided, because of the possibility of cross-reactivity, and an alternative antibiotic, e.g., clindamycin, vancomycin, or ciprofloxacin, is prescribed. This may be a short-term risk-avoiding strategy during surgery, but the long-term consequences are overuse of these agents and an increase in serious hospital infections by pathogens such as Clostridium difficile and vancomycin-resistant Enterococcus, with an accompanied rise in healthcare use and costs. In fact, the overuse of non–ß-lactam antibiotics because of reported penicillin allergy has been labeled a public health problem.6–8 In this review, we provide an evidencebased and practical approach to patients with presumed ß-lactam allergy admitted to the operating theater and give guidance on the selection of alternative antibiotics based on cross-reactivity patterns.

What started with a vision that “no patient shall be harmed by anesthesia” quickly became what is now the Anesthesia Patient Safety Foundation. Join us for a review of patient safety initiatives and discover the steps necessary to reduce adverse events in the perioperative period. Strategies for closing the gap between the perception of safety and the reality that much remains to be accomplished will also be discussed.

Patient Safety Highlights 2017 - Hand Hygiene, OR Attire, OR Traffic: What is the Data and Can We Make a Difference?

The current prevalence of HCAIs and SSI are major public health concerns. Healthcare-associated infections (HCAIs) increase morbidity and mortality, pain, cost, and duration of stay. Sepsis following surgical injury remains a growing and worrisome problem following both emergent and elective surgery. Anesthesia providers play a central role in several key process measures, including timely and appropriate administration of prophylactic antibiotics and maintenance of core normothermia. Patient contact and healthcare provider attire and hand contact can be important sources of intraoperative bacterial transmission events associated with postoperative infection development. Despite progress in recent years in the surgical management of patients (ie., ERAS programs), postoperative complications are frequent. The use and knowledge of the literature is important for awareness of current findings and implementation of advances in surgical infection and patient well-being.

However, certain patient/procedure combinations may not be suitable for freestanding ASCs where prolonged recovery or the frequent need for overnight observation may be encountered. Examples such as the obese child with severe OSA, the very young infant with history of prematurity or the child with moderately severe asthma will be discussed. 2016 Refresher Courses in Anesthesiology - August - Positioning Problems You Hope to Never Encounter

This Refresher Course describes how positioning anesthetized or sedated patients for surgery and diagnostic procedures is an underappreciated, yet extremely important clinical practice that dramatically impacts surgical and patient outcomes. 2016 Refresher Courses in Anesthesiology - October - My Laryngoscopy Failed. What Next?

This Refresher Course describes how airway management is perhaps the skill set that best defines our specialty; yet when difficulties are encountered, we often persist with ineffective strategies. When serious injuries or complications are averted, we risk becoming complacent, reinforcing suboptimal behavior; when they are not averted, the outcomes are often disastrous, damaging the lives of patients and their families and significantly impacting those who were entrusted with their care. Near misses are not uncommon, but when our patients escape injury, we may fail to learn from these situations. It behooves us to pay attention and become better prepared. Multiple ineffective interventions waste time, risk injury, and may convert a failed intubation into inability to oxygenate a patient.

Drug Errors in Anesthesia

Administration of potent anesthetic drugs with small intrinsic margins between effective doses and overdose requires constant vigilance. Anesthesiologists are acutely aware of the numerous claims related to drug errors included in the ASA Closed Claims Database and are familiar with the lexicon that classifies mechanisms of drug errors. Therefore, anesthesiologists know that the most common distinct mechanisms of error are substitution, insertion, and incorrect dose. They also must recognize that an incorrect dose is most likely to produce serious or even fatal consequences. Physicians need to regularly review the OR procedures to ensure safe drug use, and be alerted to the change before the drugs are provided and stocked in the OR.
