Drug Errors in Anesthesia

Administration of potent anesthetic drugs with small intrinsic margins between effective doses and overdose requires constant vigilance. Anesthesiologists are acutely aware of the numerous claims related to drug errors included in the ASA Closed Claims Database and are familiar with the lexicon that classifies mechanisms of drug errors. Therefore, anesthesiologists know that the most common distinct mechanisms of error are substitution, insertion, and incorrect dose.
